Transition to Changing Lives SEN

At Changing Lives Independent School, we understand that moving to a new school can be a significant step for both students and families. Our carefully structured transition programme ensures that every young person feels supported, prepared, and confident before they begin their journey with us.

Personalised Transition Process

Assessing Student Needs
Each student’s Education, Health, and Care Plan (EHCP) is reviewed to ensure that Changing Lives SEN is the right fit. We consider:

  • Prior learning levels

  • Additional support needs

  • Personal strengths and interests

Collaboration & Consultation
We work closely with:

  • Parents & carers to understand concerns and expectations

  • Current school staff & professionals to gather insights and support the transition

  • The student, to help them feel comfortable and excited about their new learning environment

Gradual & Supported Transition
Once a place is confirmed, a bespoke transition programme is put in place, which includes:

  • Supported visits to Changing Lives SEN, where students can meet staff, explore the environment, and take part in short activities

  • Transition materials, including printed and digital guides tailored to the student’s needs

  • Staff support from their current school, where possible, to ease the adjustment

Our goal is to ensure that every student arrives at Changing Lives SEN feeling confident, familiar with their surroundings, and ready to succeed.
