Personalised Transition Process

Each student’s Education, Health, and Care Plan (EHCP) is reviewed to ensure that Changing Lives SEN is the right fit. We consider:

We work closely with:
Parents & carers to understand concerns and expectations
Current school staff & professionals to gather insights and support the transition
The student, to help them feel comfortable and excited about their new learning environment
✅ Gradual & Supported Transition
Once a place is confirmed, a bespoke transition programme is put in place, which includes:
Supported visits to Changing Lives SEN, where students can meet staff, explore the environment, and take part in short activities
Transition materials, including printed and digital guides tailored to the student’s needs
Staff support from their current school, where possible, to ease the adjustment
